The 77.8 V reverse standoff is the working voltage the line sits at under normal operation; the 125 V maximum clamping voltage is where the device turns on and clamps the spike — at 12 A peak pulse current. For a telecom line running near 48 V nominal, 77.8 V standoff gives comfortable headroom above the DC working point while keeping the clamping ceiling low enough to protect downstream electronics.
