Through-hole TO-262-3 (I²Pak, PG-TO262-3-1) with long leads. The three-pin layout matches standard TO-262 footprints; the tab is the drain. Heatsink the tab—the 104 W maximum power dissipation at case temperature assumes a good thermal interface. No isolation pad between tab and heatsink unless the system design accounts for drain potential.
